What Are the Suprahyoid Muscles? The Four Muscles That Lift Your Jawline

The suprahyoid muscles are four small muscles that sit above the hyoid bone, form the floor of the mouth, and attach to the underside of the mandible. Together they pull the hyoid up and forward. Individually they each do slightly different things. Collectively they are the muscle group that determines whether your jaw to neck transition is sharp or soft.

Almost no fitness content talks about them. Almost no ordinary activity loads them. And they are the direct target of a visible jawline.

The four muscles

1. Mylohyoid

The largest of the group. A thin, flat, triangular sheet that spans from the inside surface of the mandible to the hyoid bone, forming the actual floor of the mouth. When it contracts, it lifts the hyoid up and helps elevate the tongue during swallowing.

2. Geniohyoid

A narrow strap that runs from the inside of the chin (the genial tubercles) to the front of the hyoid. It pulls the hyoid up and forward. When toned, it directly influences the projection of the chin from the neck.

3. Digastric

Two muscle bellies joined by a tendon that loops through a sling attached to the hyoid. The anterior belly pulls from the chin, the posterior belly pulls from behind the ear. Together they elevate the hyoid and depress the mandible during opening.

4. Stylohyoid

A thin muscle running from the styloid process behind the ear to the hyoid. It pulls the hyoid up and back during swallowing.

Combined, these four muscles form the muscular platform under the jaw. Their tone determines whether that platform is taut or slack, and therefore how the jaw to neck line appears.

What suprahyoid tone looks like from the outside

You cannot see the muscles directly. They are covered by skin, fat, and the platysma. But their tone shows up in two visible ways:

  1. The angle between the underside of the jaw and the front of the neck. Toned suprahyoid muscles pull this angle toward ninety degrees. Weak suprahyoid muscles let this angle open up toward one hundred twenty or more.
  2. The firmness of the tissue just behind the chin. Toned suprahyoid muscles keep this area supported. Weak suprahyoid muscles let it sag into what people call a soft chin or a double chin without fat cause.

This is why slim people with poor suprahyoid tone can still look like they have a weak jawline. The bone is fine. The fat is low. The muscle platform under the jaw is not doing its job.

Why the suprahyoid group atrophies in modern life

These muscles get almost no daily stimulus:

  • Chewing does not train them. Chewing loads the masseter and temporalis, which are above the mouth, not below it.
  • Talking does not train them. Speech uses tongue muscles primarily.
  • Neck exercises do not train them. Chin tucks and cervical strengthening target the platysma and neck flexors, not the suprahyoid group.
  • Facial exercises do not train them. Face yoga and expression exercises target expression muscles.

The only routine daily activity that meaningfully loads the suprahyoid group is forceful swallowing, and modern eating requires very little of it. So the group atrophies from disuse across the population.

What actually loads them

The suprahyoid muscles are hard wired to a specific reflex: when the upper airway is under threat of collapse (during forceful inspiration, during swallowing, during snoring), these muscles fire reflexively to stabilize the airway. This is called the upper airway dilator reflex.

The reflex can be triggered on demand by breathing hard against resistance. Every forced inhalation causes the suprahyoid group to contract reflexively at high effort. Repeated daily, this loads the muscles progressively the same way any strength training loads a muscle. Over weeks, they hypertrophy.
